To clear the template do the following:
1. Start CAPITAL Business Manager
2. Click on "Maintenance" (bottom left corner)
3. Click on Customize (icon near top left)
4. From the menu select Install Settings|Transactions
5. Click on the button that reads: Transaction Sequences & Forms...
6. Double click on "Statement Transactions" (this is just above the middle of the list).
7. Where it reads "Form Name" press <spacebar> to clear it.
8. Press Save and Save again.
9. Press Save (top right corner).
Now, close all running copies of CAPITAL Business Manager. Restart CAPITAL Business Manager on your computer first.
Changes should now take effect.
